A 35 yr old female labour by occupation came to opd with 

CHEIF COMPLAINTS:
She came with complaints of fever ,headache,bodypains, chills since 6 days along with shortness of breath.
History of presenting illness.
Patient was apparently asymptomatic six days back .suddenly she developed fever, chills , along with dyspnea
 Fever that is continuous,low grade, temperature is raising at evening. Headache is on frontal region.
No nausea , vomiting,blurring of vision.
Occasionally she had difficulty in passing stools and have blood in stools.
Past history 
Know case of HIV since 2005 
History of tubectomy 16 years ago
No DM, No HTN, No asthma,no epilepsy, No TB

Personal history
Loss of appetite
Mixed diet 
Irregular bowel movement
Normal micturation
Sleep inadequate
No allergies
Occasionally alcoholic
No drugs allergies 
 
Family History
Father had TB 20 years ago
No know case of DM ,HTN

Mesntrual history
 Age of menarche 13
Menstrual cycle :past 5/ 

General examination
 
Conscious, coherent
Co-operative, moderately built 
No pallor
No cynosis
No icterus
No clubbing of fingers
No pedal edema
No lymphadenopathy
No malnutrition
Mild dehydration

Vitals
Respiratory rate 19 / min
Bp 110/70
Spo2 99

History of present illness: the patient was apparently asymptomatic 1 week back. She had fever since 1 week and said that there was increased temperature at night and diurnal variation in temperature.
